Abstract

Inequality between high health complaints and low formal medical facility utilization poses a serious challenge to the effectiveness of the regional health information system. This study aims to analyze the influence of health complaints, JKN membership status, and self-medication practices on outpatient service utilization in Central Java Province in 2024. This research, employing an analytic epidemiological study design through an ecological approach, involved 35 district and city administrative areas. Secondary data collection utilized a total sampling technique based on the 2024 Central Java Province Health Profile. Hypothesis testing applied multiple linear regression analysis to measure the effect of independent variables on formal medical facility visits. Multivariate computation proved the existence of a simultaneous effect among the three independent variables on outpatient service utilization with a probability value of 0.029 and a determination coefficient of 0.25. Partial testing revealed an anomaly of a negative effect of health complaints on medical seeking decisions with a significance value of 0.009 and a coefficient of -0.479. The increase in morbidity burden actually reduced visit probability due to the low perceived severity of illness among the public. Insurance membership status and the dominance of self-medication behavior proved to have no significant partial effect. The cultural homogeneity of self-medication successfully suppressed the appeal of free medical facilities while reducing the probability of public queuing. Health policy authorities must design intervention programs to improve service quality and educate on the dangers of self-medication to save data quality in regional medical record information systems.
