A Root Cause Analysis of Purification Failure in BPJS Outpatient Claims Process at Hospitals

Abstract

Cases of failed purification require special attention.This study aims to identified causes of purification failures in data validation process for outpatient BPJS health claims. This is expected to improve accuracy and efficiency of management in outpatient BPJS claim process. Qualitative research with a case study approach. Results identified purification process flow from patient data input process with MySiloam application, claim data collection process with HOPE application and EMR application, and coding process with Siblink application. Purification failures occurred in October as many as 20 (0.24%), November as many as 12 (0.16%), and December as many as 47 (0.65%). Causes of failed purification were card number errors (25 (53.19%), incorrect SEP date errors (19 (40.43%), and unregistered SEP numbers (3 (6.38%). Fishbone diagram analysis of human factors, inappropriate data input (typing errors in SEP numbers, BPJS card numbers, SEP dates that do not match patient documents), lack of training and understanding of BPJS claim validation procedures, and high workloads. Machines, disruptions in hospital management information system and healthcare BPJS systems, network instability and poorly integrated applications, system errors such as inputted data not being able to be synchronized with the BPJS system, and system being unable to detect registered SEP numbers. SOPs are unclear or inconsistently implemented, and there is no specific SOP for data validation process. There is no double-check process and no automatic verification feature to double-check for errors before submission. Material (data), mismatches in patient data between medical record documents and data input into the BPJS system.
